What is a bill of sale?

A bill of sale is a written record of a private sale: who sold what to whom, when, and for how much. For a vehicle it also records the VIN and the odometer reading. Both sides sign it and keep a copy — it is your proof the sale happened on those terms.

Does a bill of sale transfer a vehicle title?

No. The signed vehicle title is what transfers ownership; a bill of sale is the supporting record of the sale. Title, tax and registration rules vary by state, so check with your local motor vehicle office. This is a general-purpose form, not legal advice.

What does "as is" mean?

It means the buyer accepts the item in its current condition, with no warranties from the seller about its future condition or performance. Leave the as-is box unchecked and the sentence will not print.
